Heme Health has created the Following Terms and Conditions, separately for App/Portal users and for Website users.
HEME APP TERMS AND CONDITIONS (FOR INDIVIDUAL USERS)
BACKGROUND:
Heme Health ("Company") has developed the Heme App and Heme Web Portal, defined together as the App (“App”), which allows individuals to store and access their medical information in a centralised ****location (defined below).
INTRODUCTION:
These Terms and Conditions (T&Cs) set forth the standards of use of the App and shall form your legally binding agreement with us once accepted (the “Agreement”). The words (“You”) or (“User”) or (“Your”) as used in these T&Cs mean to include and refer to all individuals accessing or using the App for any reason.
DEFINITIONS:
- “Heme App” or “App” is our mobile application on iOS and/or Android and our Web Portal, which include elements of a range of Heme products, the scope of which may change from time to time.
- “Lab” is the diagnostic centre the User has contacted and availed healthcare services.
- "Medical Records" is the electronic medical report or other health data generated by the Lab for the User. The Medical Records shall be uploaded on the App in order for the patient to view, store and download the relevant Medical Records.
- "Services" is collectively the various services offered by the Company. These services may change from time to time on the App.
ELIGIBILITY:
You represent and warrant that:
- you are a resident of India or the UK;
- you are 18 years or older, or at the legal age to form a binding contract in the jurisdiction of your residence;
- you have the right and authority to legally bind yourself or your family members or your company, as applicable;
